What is the Extended Care Program?
The Extended Care Program offers a four week at home program for patients who require rehabilitation following knee or hip replacement surgery. The program is goal oriented, and provides the patient with six physio visits, four nursing visits and an outpatient review by our rehab specialist at the end of the program. Participants benefit from an individualised program planned to suit their ability. For those patients who have undergone breast surgery, urological surgery or require management of complex dressings using VAC therapy, the program is planned to suit participant’s ongoing care needs as indicated by their treating doctor. The program continues the high standard of care offered in hospital at home.
Extended care program services St Vincent’s Private Hospital Sydney can now provide you with care at home for needs such as:
- General rehabilitation at home
- Rehabilitation after your hip or knee replacement
- Complex dressings that require VAC therapy
- Care after urological surgery
- Care of patients undergoing breast surgery, eg. drain management at home

Do you qualify?
Your care delivery at home will be discussed with a member of the Extended Care Program whilst you are in hospital.
Criteria to participate include:
- Your consent
- Residential address within an hour’s drive of St Vincent’s Private Hospital Sydney
- Access to a telephone
- Safe home environment
Program plan and review
You will provide with a detailed plan of your care before you leave hospital. The Nurse and Physiotherapist will contact you prior to their visit to organise a suitable time.
